Customer Services Application Forms

Dear Customer,

For your convenience, you will find the forms that you may need in case you wish to submit one of the applications provided by the Electricity and Water Authority through the Customer Services Directorate, and some of these forms can be filled out in PDF format that the user can fill out and save.

Some of forms are fillable in PDF format - normally PDF is used as a final publishing format, however PDF gives the option to be used as an entry form that can be edited and saved by the user.

Just fill in the required form than print it in either color or black (monochrome) - either option will be accepted.

These are the required standard forms for most services provided by Electricity and Water Authority through customer services directorate

Customer Services Applications Forms
Electricity, Water, Municipal Rates Agreement & General Services Request (required for all Applications)
Used for Service Reconnection, Transfer of Names, Change of Tenancies, Details Update or Modification Request, Electricity / Water Service Disconnection Request and Data of New Supply
Electricity Supply Request (for New supply, Additional Load supply ,Temporary & Split of supply) The Application form must be stamped by the Seal of Authorized Electrical Contractor
Extension of Supplementary Details for Electrical Installations The Application form must be stamped by the Seal of Authorized Electrical Contractor
Electricity Installations Completion & Test Certificate (For meter(s) installation) The Application form must be stamped by the Seal of Authorized Electrical Contractor
Water Supply Request (for New and Additional supply) The Application form must be stamped by the Seal of Authorized Plummer
Infrastructure Fees exclusion Request

Inspection of periodic electrical installations Certificate The Application form must be stamped by the Seal of Authorized Electrical Contractor
Water Sub Meters Request Form The Application form must be stamped by the Seal of Authorized Plummer

Customer Pledge to not repeat of Electricity / Water installations violation & Meters tempering irregularities
