Survey Certificate A certificate detailing the property area and surrounding roads and lands.
Water Supply Request Form A form filled and signed by the customer or their authorized representative, approved by an authorized plumber, with the plumber’s valid license and stamp.
Aerial Map An aerial photo showing the property and its area.
Title Deed A document issued by the Survey and Land Registration Bureau showing the name of the owner, property number, and ownership transfer date.
Plumber Compliance Form A form created to ensure the customer complies with water connection regulations, approved by an authorized plumber, and followed by a field inspection for meter installation.
