Customer Services Application Forms
Dear Customer,
For your convenience, you will find the forms that you may need in case you wish to submit one of the applications provided by the Electricity and Water Authority through the Customer Services Directorate, and some of these forms can be filled out in PDF format that the user can fill out and save.
Some of forms are fillable in PDF format - normally PDF is used as a final publishing format, however PDF gives the option to be used as an entry form that can be edited and saved by the user.
Just fill in the required form than print it in either color or black (monochrome) - either option will be accepted.

Electricity & Meter Services Agreement
|
This agreement allows customers, through a licensed contractor, to request electricity and meter services, such as new connections and meter installations. It ensures proper processing and adherence to EWA guidelines.

Water Supply & Service Agreement
|
This agreement allows customers to request water supply services, including new connections, changes in connection size, modifications to the supply system, and the installation of water sub-meters. It requires details from both the customer and a licensed plumber to ensure compliance with EWA standards.

Account Management Agreement
|
This agreement enables customers to manage their electricity and water accounts, including opening or closing accounts and updating personal details. It also allows premise owners to notify EWA of a tenant's departure or to guarantee another account.

Technical Services Application
|
This form enables customers to request various technical services for electricity and water, such as meter testing, relocation, or removal. It also includes services related to street lighting, such as the installation, relocation, or removal of poles or wall-mounted lights. Customers can provide their details and specify the required service to ensure efficient processing.

(Deposit/Credit) Refund Request
|
This form allows customers to request a refund of deposits or credit amounts by providing their account and ID details, as well as the beneficiary's information for the transfer. It ensures accurate processing in accordance with EWA terms and conditions.

Electrical Installations Inspection Form
|
This form is used to confirm the visual inspection of electrical wiring and earthing. It must be stamped by a licensed electrical contractor.

Plumber Compliance Form
|
This form is used to verify that the plumbing work complies with EWA standards and regulations. It must be stamped by a licensed plumber.

Extension of Details for Electrical Installation
|
This form is used to provide additional details for electrical installation and must be submitted alongside the Electricity & Meter Services Agreement. It also requires a stamp from a licensed electrical contractor.
